Scheduling Procedures

Scheduling an independent medical evaluation from MEI is a breeze. Whether you call, e-mail, fax, or contact us on our website, being prepared to provide the following information will make it even easier.

 

Here's what we'll need to know when you contact us.

• For the examinee:
  • Name & address
  • Claim number
  • Phone number
  • Date of birth
  • Date of loss
  • Social Security number
  • Type of injury
• For the attorney (when applicable):
  • Name & address
  • Phone number
• Type of Claim:
  • (worker's compensation, casualty, personal injury, long-term disability, etc.)
• State of Jurisdiction

Also, when you schedule an exam, please let us know:

• If you need any assistance from MEI such as translation or transportation.
• Any confirmed or potential hearing dates.
• Whether you will need the physician to give testimony at either the trial or in a deposition.
• If you are rescheduling an appointment.
• Special requirements for expediting the final report.
• If the medical records are extensive and would require more lead time.
• If the examinee's attorney has placed any restrictions on the evaluation.
• If the examinee's attorney or doctor plans on attending the exam.

Once you have scheduled the exam, a few details can ensure that everything will go smoothly. Please send all correspondence and medical records soon after you've scheduled the exam. Receiving the records two weeks prior to the exam date allows us to better prepare the physician.

Cancellation Policy

Giving you Access to the Best Physicians

 

To make sure we can always offer you the services of top-quality physicians, we need to compensate them for lack of attendance and short-notice cancellations.

 

It's important to note that we can only accept cancellations from the referring party. The examinee may not cancel the exam directly.

 

Here's how you can avoid additional fees.

 

Standard late cancellation is three business days with exceptions (see chart below)
